Upon booking an appointment, you will be emailed a link to a secure client portal. Once you are logged in, you will electronically complete all intake paperwork as well as electronically sign the Disclosure Statement, Consent to Treatment and HIPAA Notice of Privacy Practices.
If a computer is not accessible to you, we can discuss how else to complete the necessary paperwork prior to the first session.
All paperwork must be completed at least 24 hours prior to the first session.
